Geocoding and spatial analysis of data describing populations and health events are important methods in health social science now carried out using GIS technology. This commentary considers Nancy Krieger’s work on health disparities in light of the various ways individuals and organizations use geocoded population and health data: analyzing spatial patterns of health and disease including health disparities, aggregating data spatially, assessing health status of individuals based on characteristics of aggregates, modeling neighborhood contextual factors affecting health, designing observation and intervention studies, and delivering health interventions and services. The extent to which her work addresses each of these purposes is considered. The strengths and limitations of the research including choice of spatial analytic units and techniques as reported in the published work are discussed. Krieger’s work, with her colleagues, has used standard methods of spatial analysis to raise the profile of GIS and spatial analysis in the public health community.  相似文献

Despite the increasing public profile of same-sex issues, health policies are often shaped by heteronormative assumptions. The health concerns of lesbian, gay, bisexual, transsexual/transgender, two-spirit, intersex, queer and questioning (LGBTTTIQQ) people are complex and require broadening from an often exclusively sexual health and risk focus to a more holistic approach. In this context, this paper illustrates how a critical feminist geography of health, with its focus on the mutual construction of gender relations, space and place, potentially enhances and extends current understandings of public health policy and practice. Moreover, the use of a policy lens foregrounding gender and other power relations suggests that feminist research and coalitions facilitate participatory processes that address “the politics of discourse.” In particular, public health nursing practice can enhance the construction of spaces of resistance that challenge heteronormative discourse through research strategies focused on sexual minority communities’ health experiences and their visions for supportive care. In this respect, two strategies consistent with public health priorities to increase knowledge and participate in alliances are described. Ethnographic research with childbearing lesbians demonstrates that attention to institutional dynamics that foster safe spaces can facilitate access to public health services. Public health nurses’ involvement in community coalitions can enhance dissemination of community knowledges. The implications for gender inclusive and place-sensitive public health nursing practice include the development of sensitive educators, meaningful educational curriculum and related program planning, explicit policies, community partnerships and political leadership in institutional and research venues.  相似文献

The professional formation of German geography teachers is advanced both at university level and for all types of schools. There is a long tradition of teaching geography as a separate independent subject in classes 5–13, whereas it is an integrated subject in classes 1–4 and only in some states, on the secondary level. Speaking for all schools and all ten federal states, it can be maintained that there is a minimum of one geography hour per week, sometimes up to two hours. However a certain decline of classroom hours has occurred during the last 30 years (which most other subjects suffered as well). As most geography teachers have another subject (or two), they have a tendency to identify as a maths teacher, for example, because of the higher general esteem the second subject may have. Still, public opinion considers geography to be of general importance for an educated citizen. This has effectively influenced the curricula, emphasizing cognitive insight in geographical aspects, topography and general understanding of man-land relationship. A higher level of critical insight, planning issues and higher intellectual objectives can be reached with geographical topics.  相似文献

To put on the map: according to the Oxford English Dictionary, whatever is so placed occupies “an important or prominent position” and is “of some account or importance.” In this brief commentary, I take the opportunity to reflect on several articles by medical/health geographers and others regarding our Public Health Disparities Geocoding Project and my research, as a social epidemiologist, on the myriad ways racism can harm health. In both cases, my work on these topics has been motivated by my desire, starting a quarter of a century ago, to put health inequities on the map: as a topic of public concern, research, and action. The insightful and informative points and questions raised by the geography-oriented articles underscore the importance of bringing the best of our respective disciplinary insights and expertise to illuminate the profound connections between social justice and public health, between our bodily truths and the body politic, so as to further the goal of embodying equity––in an ecologically sustainable world.  相似文献

Geography is enjoying a period of unparalleled visibility, driven by the growing use of geographic methods and concepts across the sciences and humanities—the so-called spatial turn—and the pervasive use of geospatial Web technologies and their concomitant influence on society, especially the phenomenon of volunteered geographic information (VGI). The field of public health is beginning to harness spatiality with gusto; however, the geospatial Web and its social phenomena are underexplored in this context even though they may be particularly useful for public health enquiry, especially in low-resource settings that lack traditional data collection mechanisms. A case study framed within these two current phenomena is presented to illustrate the influence of geography and its potential for addressing the data-divide—the disparity in availability of data for scientific enquiry and decision-making most felt in low-and middle income countries. A facilitated VGI data collection initiative collected public health-related injury data in Cape Town, South Africa, as a pragmatic alternative given the lack of data from traditional sources. Emergency medical services personnel interacted with a GeoWeb interface to volunteer their informed opinions of high-incident injury locations. Previously unrecorded injury location data were created, and combined with traditional injury data for use in an ongoing study examining the environmental determinants of injury in this setting, which speaks to the possibility for hybrid authoritative/asserted data collection strategies. This study speaks to the growing influence of geography and one of its driving forces, the techno-social revolution in geospatial technology and data. Future work should continue to examine their potential to address the data-divide.  相似文献

The article advocates an articulation of cultural geography and art history, and in this perspective focuses on the analysis of the primitivist movement and particularly on Gauguin's work and personal itinerary. Primitivism introduced artefacts of ‘primitive’ people into the history of Western art and signalled a change in the relationship between the West and the ‘Other’ and ‘Elsewhere’. This reversal of values has a major geographical dimension. Primitivism manifests the contradiction-rife colonial ideology, but can also challenge colonization. Tourism, which is, in the case of Tahiti, directly linked to Gauguin and to his myth, inherited a lot from primitivism, in terms of hopes and ambiguities. Conversely, primitivism casts light on geographical features of these places, instituted as ‘Elsewheres‘ by the West, and visited, even transformed by painters, colonizers and tourists. Geographic knowledge is usually understood as the knowledge produced by geographers. Yet, it is also produced by people outside of the academia. But as Western science claimed for its exclusive self the status of ‘knowledge’, others have long been denied the production and possession of a true knowledge, and recognised merely the mastering of practical skills. The binary opposition between scientific and other forms of knowledge has been central to the construction of academic geography until the late 1980s, when postmodern thought cast some doubt on the universality of scientific knowledge. This led to critical analyses of academic geography, revealing its situatedness, as well as to a new interest for the geographies of the ‘Others’. Examining how geographers have dealt with other knowledge sets so far, and how they have labelled them, this paper argues that other geographies should be given more attention. This means focusing on the knowledges themselves, and considering implementing a true dialogue between these and academic geography. Water–rock interaction is one of the prime factors affecting the fluoride contents of surface and groundwater. If fluoride concentration of drinking water has been neglected, excess fluoride can cause serious dental and medical problems on human health, which is well known at Golcuk-Isparta region. In the research area, Egirdir lake, Golcuk lake and surrounding springs have been utilized as drinking water sources. Golcuk lake water and surrounding groundwaters have high fluoride content (1.4–4.6 mg/l), which is above the WHO standards. Fluoride is predominantly supplied by dissolution of fluoride within the fluormicas of volcanics during the circulation of water. Fluoride concentrations of waters have shown variations for dry and rainy seasons depending on the degree of interaction between groundwater and volcanic rocks. It tends to decrease in rainy seasons and increase in dry seasons for all years. In this study, temporal variations and spatial distribution of fluoride concentration in public water system of Isparta were investigated to get benefit using GIS techniques from1990 to 2003 years. Extremely fluoride concentrations were measured in the public water system in 1990 at almost every district of the city. In 2003, fluoride content of the public water system decreased in some district of city due to drinking water has started obtaining from Egirdir lake in 1995. The fluoride contents of Isparta drinking water ought to be modified with suitable mixture of lake waters and groundwater point of view to health impact.  相似文献

Low birth weight (LBW), defined as a live birth weighing <2,500 g, is a significant public health problem in the United States, especially a few states including Georgia. Although much work has been done to study the epidemiology of LBW in various regions, the spatial–temporal patterns of LBW prevalence in Georgia remain unclear to a large degree. This paper investigates the temporal trend of LBW rates over a time span of 11 years and the spatial clusters of LBW prevalence in the state of Georgia at the county level. Comparison is also made between race and gender groups, and between county groups of different socioeconomic statuses to uncover disparities. Results showed a steady and prevalent increase of LBW rate in the state over the study period. Three counties and two county clusters with significantly higher LBW rates than the state rate were detected for 1999–2001, while one more county and two more county clusters of high LBW rates were detected for 2007–2009. More urbanized counties were found to have a relatively lower LBW rate when compared with the less urbanized ones as groups. The findings from this paper are expected to provide valuable insights to better understanding the etiology of LBW and more effective allocating prenatal health care resources in the future.  相似文献

Recent years have seen academic geographers engaged in a series of debates over the current state of the discipline, its ‘relevance’ to others in the social sciences, to policy-makers, and to those studying geography at school age. This short critical review builds upon an issue raised in this journal [Thrift, N., 2002. The future of geography. Geoforum 33, 291–298], namely the role of geographers as public intellectuals. After reviewing the different ways in which the notion of public intellectuals has been understood, the paper turns to geography’s representations and to its publics. The paper concludes by arguing for an appreciation of the full range of ways in which geographers call forth publics through a range of representational strategies. It suggests that regardless of how geographers perform publicly and intellectually, two things are perhaps worth remembering: it is in the interest of geographers to name what they do as geography and to name themselves as geographers.  相似文献

The ability to explicitly represent infectious disease distributions and their risk factors over massive geographical and temporal scales has transformed how we investigate how environment impacts health. While landscape epidemiology studies have shed light on many aspects of disease distribution and risk differentials across geographies, new computational methods combined with new data sources such as citizen sensors, global spatial datasets, sensor networks, and growing availability and variety of satellite imagery offer opportunities for a more integrated approach to understanding these relationships. Additionally, a large number of new modelling and mapping methods have been developed in recent years to support the adoption of these new tools. The complexity of this research context results in study-dependent solutions and prevents landscape approaches from deeper integration into operational models and tools. In this paper we consider three common research contexts for spatial epidemiology; surveillance, modelling to estimate a spatial risk distribution and the need for intervention, and evaluating interventions and improving healthcare. A framework is proposed and a categorization of existing methods is presented. A case study into leptospirosis in Sri Lanka provides a working example of how the different phases of the framework relate to real research problems. The new framework for geocomputational landscape epidemiology encompasses four key phases: characterizing assemblages, characterizing functions, mapping interdependencies, and examining outcomes. Results from Sri Lanka provide evidence that the framework provides a useful way to structure and interpret analyses. The framework reported here is a new way to structure existing methods and tools of geocomputation that are increasingly relevant to researchers working on spatially explicit disease-landscape studies.  相似文献

This paper traces changes in the political ecology of insects and chemicals in US public housing since Congress founded public housing in 1937. Drawing upon the literature of critical geographies of home, urban political ecology, and medical history, it argues that the constitution of “public” and “private” space within public housing was deeply entangled with pest control practices there. Prior to 1945, reformers treated the housing as a commons, in part compelled by the mobility of bedbugs and the pesticide used to combat them, both of which were seen as serious health threats. Managers were also motivated by social welfare ideologies, while residents eagerly assisted with communal control policies in order to achieve freedom from the health insults of bedbugs. Following 1945, however, new synthetic pesticides like DDT seemed to stay safely within one apartment unit, encouraging housing managers to abandon community-oriented pest control practices. Meanwhile, curtailed budgets, particularly after the Housing Act of 1949, left the infrastructure of public housing to decay, rendering units more physically permeable even as managers neglected the communities there. The new pesticides nearly eradicated bedbugs, but tenacious populations of German cockroaches blossomed thanks to the permeable buildings and synthetic pesticides. Residents grew increasingly resistant to pesticide use as they observed that cockroach populations went unabated. The paper serves as a case for applying political ecology frameworks to domestic spaces, and also argues that housing quality and domestic pesticide use are not merely private responsibilities but should be regarded as environmental justice issues.  相似文献

The viewpoint that most health problems are environmentally related has always been promoted by medical geography. In recent times, concern about environmental degradation has been of high priority for many national and international organizations as ‘we human beings become a threat to our planet’. There is also a need to specifically focus attention on the impact of so-called development efforts on the health of women and children, in particular those living in highly marginalized regions which often contribute substantially to the overall development of their countries. This paper uses the concept of vulnerability as a departure point from which to map the health risks to children in Igbekun area of Ondo State, the ‘transgressive muddy’ oil-rich coastal area of Nigeria. The paper attempts to highlight the poor quality of life and health of children in particular, and how it is becoming much more difficult to practice health intervention programmes, especially oral rehydration therapy (ORT) in the area. The Igbekun area is an unhealthy and harsh environment, complicated by the devastating effects of oil drilling in recent years. The siltation and sedimentation of the creeks and lagoons have serious health dimensions as human faeces float in and around villages because the ‘bush toilets’ can no longer be reached. The on-going environmental crisis has also deprived women of their traditional means of livelihood; fishing. There is a relatively high level of ignorance (compared with other women living in the same local government area (LGA) of causes of the commonest diseases such as diarrhoea, acute respiratory infection (ARI) and measles. A majority of the women have never heard of family planning, immunization against diseases and interventions such as ORT. Promotive health care services, especially those that strive to reduce the high mortality of children have passed the mothers by. It is also becoming impracticable to promote ORT because of the widespread ‘salty and oily’ water supply in the area. The greatest concern of the women now is that oil drilling activities have further amplified their stressful conditions especially their health and that of their children. Unfortunately, their plight is not acknowledged by the Nigerian authorities and the oil companies are least concerned about alleviating the poor health status of the community, in particular the most vulnerable groups, children under the age of five. Summary The ∼ 150 km3 (DRE) trachytic Campanian Ignimbrite, which is situated north-west of Naples, Italy, is one of the largest eruptions in the Mediterranean region in the last 200 ky. Despite centuries of investigation, the age and eruptive history of the Campanian Ignimbrite is still debated, as is the chronology of other significant volcanic events of the Campanian Plain within the last 200–300 ky. New 40Ar/39Ar geochronology defines the age of the Campanian Ignimbrite at 39.28 ± 0.11 ka, about 2 ky older than the previous best estimate. Based on the distribution of the Campanian Ignimbrite and associated uppermost proximal lithic and polyclastic breccias, we suggest that the Campanian Ignimbrite magma was emitted from fissures activated along neotectonic Apennine faults rather than from ring fractures defining a Campi Flegrei caldera. Significantly, new volcanological, geochronological, and geochemical data distinguish previously unrecognized ignimbrite deposits in the Campanian Plain, accurately dated between 157 and 205 ka. These ages, coupled with a xenocrystic sanidine component > 315 ka, extend the volcanic history of this region by over 200 ky. Recent work also identifies a pyroclastic deposit, dated at 18.0 ka, outside of the topographic Campi Flegrei basin, expanding the spatial distribution of post-Campanian Ignimbrite deposits. These new discoveries emphasize the importance of continued investigation of the ages, distribution, volumes, and eruption dynamics of volcanic events associated with the Campanian Plain. Such information is critical for accurate assessment of the volcanic hazards associated with potentially large-volume explosive eruptions in close proximity to the densely populated Neapolitan region. This paper evaluates the impact of recent shifts in the geopolitical outlook of the United States—specifically the ‘Global War on Terror’ and its domestic ramifications—on the prior project of reconfiguring the US state to an after-Fordist ‘workfare’ paradigm. In particular, the paper attempts to situate recent developments in the reconfiguration of the American welfare state within the context of debates over the ‘exceptional’ nature of US politics post-9/11. The extent to which this has had a bearing on work practices, welfare provision and workfarism is investigated, citing examples from the post-9/11 era. On the basis of such examples it has been suggested that the project of reconfiguring the US welfare state and labour relations does overlap and intersect with the emergency practices that the US state has instituted domestically as part of the War on Terror. However, the reconfiguration of the US welfare state towards a neoliberal or workfarist paradigm is, we argue, largely part of the ‘normal’ (as opposed to exceptional) fabric of US politics. The project of reconfiguring the US welfare state is ongoing and largely autonomous (although not entirely removed) from the use of exceptional practices in relation to the US security state. Investigation of this relationship, we assert, provides a potentially fruitful ‘new direction’ for both critical geopolitics and political economic geography, and grounds for interaction between the two.  相似文献

This review paper aims to offer a contribution to debates over theory and subject for political geography. Following a brief review of histories of political geography, the main (though not exclusive) focus is on the way that political geography may confront ‘globalization’ and the multiplicity of flows that constitute ‘cyberspaces’. Notwithstanding the consequences of the resulting transformations, the paper argues that a number of traditional subjects of political geography should remain central to the field. In particular, it is argued that a degree of state-centric focus continues to be a valuable critical project. However, such a focus needs to be supplemented by a stress on the dialectical relationships between the state, territory, culture and economy. The approach taken to this in World Systems-Theory is critiqued and some alternatives are explored. In these explorations the paper also argues for an increased engagement and cross-fertilization between political, economic, social and cultural geographies, and with critical work in political science and international relations.  相似文献

Conclusions: the Future Relevance of Medical Geography in the Third World This article has deliberately ranged widely and suggested various research themes to which medical geographers interested in health and development might turn their attention. The International Geographical Union Commission on Health and Development established in 1988 suggested a research agenda which includes many of them (see IGU Commission on Health and Development, Circular Letter No. 1 published in GeoJournal 17, 4, 659–660 (1988)). The achievement of even part of such an agenda will call for close international collaboration in research amongst medical geographers and allied disciplines.Increasingly, medical geographers are gaining applied experience in health and health care in the developing world. They are obtaining breadth and depth of knowledge and are now, for example, cogniscant of financial matters such as those involved in the cost explosion in health care worlwide which has particularly sinister implications for the health of Third World countries (Josephg and Phillips 1984). They, too, are now increasingly comfortable in dealing with epidemiological and demographic data. Geographers now no longer focus solely on phenomena such as distance decay or environments for disease. They are aware that human resources, intelligence, aspirations, attitudes and finance are all potent variables influencing successful health care and health in populations.In the future, therefore, the wide-ranging ambit of medical geography will increasingly become relevant to health and health care research in the Third World. Contributions of value will emerge both from those geographers adopting more socio-political stances and those adopting a more empiricist approach. However, it is the holistic nature of geography, with its wide academic links, and the courage of geographers to research in new topics and gain sound understanding of them which will increasingly be recognized. The days of extempore contributions to political, policy and practical debates on health and development are now largely past. Well researched, solid and sound medical geography contributions will, it is hoped, forge ahead.  相似文献

‘Community geography’ is a growing sub-field that leverages academic resources to facilitate spatial research in partnership with local communities. The Mapping Prejudice Project and the CREATE Initiative, two interdisciplinary projects at the University of Minnesota, demonstrate some of the opportunities and challenges associated with practicing community geography. Mapping Prejudice is leveraging community crowdsourcing to build the first comprehensive spatial database of racially restrictive housing covenants in the United States. CREATE is co-developing research on critical problems at the intersection of environment and equity through collaboration with community partners. These two projects incorporate a methodological commitment to place-based and historically grounded research that seeks to make knowledge in—and in relation to—a specific place. Incorporating earlier feminist and critical GIS theory, these projects have adopted an iterative research model that places under-resourced communities at the forefront of the research process. Their work produces a fluid, responsive, and co-creative approach that has the capacity to legitimate its knowledge claims through responsiveness to community needs and collective experience.

In the context of doing research on the geography of health and health care, ‘connecting’ takes on multiple meanings. First, there are the connections between research topics although the political and socio-economic realities of the various geographical contexts can lead to foci on different aspects and social and geographic processes. There are then the connections between researchers from developed and developing countries who share an interest in the geography of health and health care, who perhaps transfer or refine research methods to look at the different contexts and even different topics. Finally, in this context, there are the connections between researchers and the IGU Commission on Health, Environment and Development (HED) promoted by the HED for the past eight years through its activities. This paper uses examples from the literature and the activities of HED to illustrate these various connections. It argues that if the geography of health and health care is to continue to develop, these three sets of connections must continue to grow and strengthen.
