新生儿缺氧缺血性脑病的CT研究

摘    要:本文报告94例围产期窒息引起的新生儿缺氧缺血性脑病,其早期CT表现为水肿、梗塞和出血.缺氧缺血性脑梗塞早产儿主要表现为脑室周围白质软化,足月儿则为皮质-皮质下局部梗塞,严重者为全白质梗塞。后期改变有两侧脑室对称性不规则扩大、局限性和弥漫性脑实质萎缩、脑软化灶、脑穿通畸形囊肿、积水性无脑畸形、脑积水等。本文就CT表现与预后关系作了初步探讨,其中以脑水肿预后最佳;脑室周围白质软化Ⅲ级.全白质梗塞合并颅内出血预后最差.

Abstract:The morphology of 94 newborn infants with hypoxic ischemia encephalopathy has been analysed with CT. The early appearances on CT were classified as follows: (1) brain edema; (2) infarction; (3) intracranial hemorrhage. Periventricular leucomalacia(PVL) in severe anoxemia was only found in premature infant, whereas laminar infarct of cortex and total white matter infarction occurred in mature infant. All the brain infsrct eventually underwent liquefaction, leaving enlargement of the bilateral ventricles of the brain, porencephaly, hydranencephaly, etc. The normal proportion of CT reexamine of PVL I degree was 94. 1 per cent, but still present 29. 4 per cent of infants suffering from functional disorder of the neurimotility, because PVL could injure the astrocytes or astroblast, as a result of obstructed neurilemma. At present, CT couldn't display the foming state of the neurilemma, so appearances on CT of the brain of paralytic infants are out of all proportion to their functional disorder of the neurimotility.
